Mysti’s Adult and Family SVCS has been recognized by the State of California for its exceptional work providing sustainable housing services through the Providing Access and Transforming Health (PATH) initiative. The recognition highlights the organization’s innovative approach to helping California’s most vulnerable populations achieve housing stability and improved health outcomes across multiple counties.
The state recognition acknowledges Mysti’s success in expanding access to Enhanced Care Management (ECM) and Community Supports services across Monterey, Contra Costa, Los Angeles, and Santa Cruz counties. Through the PATH Collaborative Planning and Implementation initiative, the organization has demonstrated remarkable results in helping clients transition from homelessness and incarceration to stable housing and employment. This comprehensive approach addresses both clinical and non-clinical needs of high-risk Medi-Cal members, providing intensive coordination of health and health-related services.
Mysti’s specialized services include Enhanced Care Management for all populations, along with Community Supports such as Nursing Home Diversion, Short Term Post-Hospitalization Housing, Recuperative Care, and Respite Services. The organization’s field-based model focuses on building trusting relationships with members while providing comprehensive care that extends beyond traditional medical settings.
The organization’s transformative impact is exemplified by a client released from prison in 2023 who had struggled with substance use and felt failed by previous programs. Mysti’s team focused on building confidence while securing full-time employment and permanent housing. Working closely with case managers, the client successfully obtained a Section 8 housing voucher and received comprehensive support during a medical recovery period, including assistance with scheduling surgical care and ensuring safe recovery accommodations.
“I’m proud to say that I now have permanent housing. I have a beautiful one-bedroom, one-bathroom apartment with a great kitchen and a patio where I can cook carne asada and start rebuilding my confidence,” the client shared.
Through PATH’s collaboration with other Medi-Cal providers in the Coastal CPI collaborative, Mysti’s has significantly increased referral networks by sharing ideas and building partnerships. This collaborative approach enables more clients to access essential services, including Nursing Home Diversion, housing services, Short Term Post-Hospitalization Housing, Recuperative Care, and Respite Services. The PATH initiative has enabled Mysti’s to grow their ECM and Community Supports programs substantially, helping more people achieve their goals and improve their health through stable housing.
